A Clinical Study in Cases of Biliary By-Pass Procedures for Hepatico-Choledocholithiasis



Abstract
The clinical observation and review of 55 cases biliary by-pass procedures for hepatico-choledochlithiasis was performed, which were treated at Department of Surgery, KGH, Seoul from January 1970 to December 1982.
The results were obtained as follows;
1) The incidence of cholelithiasis in 55 cases was following orders; CBD and intrahepatic stones: 46%, CBD stones: 28%, intraheptic stones: 16% and gallbladder, and CBD stones: 10%.
2) The ratio of male to female is 1 to 18 in 55 cases.
3) The incidence was height in 30-49 years.
4) The prominant common symptoms and signs were right upper quadrant and epigastic pain(81.8% ), nausea and vomiting (54.5%), tenderness (47.2%), jaundice(43.6%) and fever and chills(40. 0%) in order.
5) Operative procedures were as follows; choledochojejunostomy: 33 cases(60%), chole-doehoduodenostomy: 20 cases(36%) and sphincteroplasty: 2 cases(4%).
6) Postoperative early complications were wound infection in 4 cases, respiratory infection in 1 case, upper G-I bleeding in 1 case and hepaticcoma in 1 case.
7) The operative mortality was 1.8% and hepatic coma was the cause of death.
8).. In lata sequalles, 2 cliolangitis was developed in choledochojejunostomy and 2 cholan-gitis in choledochoduodenostomy and peptic ulcer needing operation was not developed in any cases.
